Am I Eligible?
Residents in Alberta can receive Home Care services as long as their healthcare needs can be safely met in the home. Please contact the Lethbridge & Area Home Care Contact Center at 403-388-6380 to initiate your Home Care Application. They will submit your care plan to LFS Home Services once you’ve been approved.
After being approved, a Home Care nurse completes an intake assessment, and the following is taken into consideration to determine eligibility:
The individual may be acute, chronically or terminally ill, and require assistance.
The individual must be agreeable to the treatment and support services being provided in the home. The client is responsible for providing the necessary equipment and supplies for services.
The home environment must be conducive to an adequate standard of care and service for the individual and safety of Health Care Aides.
The individual must be under the care of a physician and be an Alberta resident, registered under the Alberta Health Care Insurance Plan.
The individual must have no immediate need for in-patient hospitalization.
Requests for services may be made by: the client, the client’s family/friends, acquaintances, guardian, home care coordinator, or a benefits worker.
Location of Service and Hours of Operation
LFS Home Services provides home care in your place of residence within our operational areas. Client service hours are 24 hours a day, 7 days a week. Office hours are Monday to Friday, 6:00 am to 10:00 pm.
Cost
If you are eligible for support under Alberta Health Services, there are no fees for AHS Authorized Personal Care Services. However, LFS Home Services does authorize private services that are subject to fees if support is a private request for assistance. As a non-profit organization, you will find our quotes reasonable and competitive. Please call 403-317-4624 if you would like to discuss private care options.
